Neonates refers to new born within the first 28days of their birth

Neonates emergency is therefore a life threatening condition that sometimes emanated at birth

The Common neonatal emergencies occur during birth or are noticed at birth eg severe birth asphyxia, jaundice, accidental birth where the baby falls or hits its head, infection, not breathing well discolouration of the neonates etc.

How can the Parent Identify an Emergency

1 If the baby does not cry at birth

2 If a baby does not suck at birth

3 If a baby runs temperature

4 Discolouration of the skin within the first 24 hours

5 If the baby breathes with difficulty or breathing very fast.etc

If any other signs are noticed the mother should seek medical attention for the baby very fast.

THREATMENT

The history of the pregnancy will be taken and such emergencies should be anticipated and prepared for the baby is placed in an incubator and monitored while treatment is on going.

ADVICE

Mothers should attend antenatal in proper hospitals Pregnancies should be planned Baby clothes should not be kept with Naphtali balls (Camphor ) baby should be born in clean environment etc

Pregnancy is the act of the formation of a baby in the womb of a woman.

This topic is important for young women who are married or sexually active to take precaution. At conception, the ovary releases the egg to be fertilized by the sperm. The fertilized ovary now travels to the wall of the uterus and gets attached.

Ectopic pregnancy is when the fertilized ovum does not get to its destination; it either develops in the fallopian tube or outside the womb. The embryo will grow bigger than the space and will eventually rupture.

SYMPTOMS

  • Spotting, the woman will start by having blood stains which will eventually increase.
  • There will be high temperature
  • The body will begin to swell starting with the face and gradually to the legs and all the
    body

TREATMENT

Treatment is basically by surgery, in most cases the affected fallopian tube may be cut off.

MATERNAL DEATH: CAUSES  AND PREVENTION

 

Maternal death refers to death of women of child bearing age. About 52,000 women die every year during pregnancy. Most of these deaths can be prevented because the causes are known.

Causes

1.     Delays ;there are three delays

Delay in seeking heath care. A situation where a woman does

not quickly seek medical advice when she is sick until it becomes an emergency.

- Delay in community infrastructure: bad road, lack of ambulance and poor transportation system. These factors make it difficult to assess the nearest health facility Delay in hospital beaurocracy: The slow system of obtaining hospital card, making payments and getting registered before the doctor comes to start consulting.

  1. Cost of obtaining medical care. In most cases women who die
    are of the low class where they have difficulty in accessing the
    medical facility.
  2. Pre-eclampsia - High Blood pressure in pregnancy.

PREVENTION

Register for antenatal early in pregnancy

save for emergency.

Eat the right food

Seek medical advise as soon as any abnormality is noticed.

Seek orthodox medical care instead of staying in church or with a native doctor.

Give    birth   in   a   recognized    health   facility   instead   of   with inexperienced local birth attendant.

Topic              The Importance of Breast Feeding

Breast milk is the best form of feeding for a newborn. For close to 20yrs, exclusive breast feeding is being advocated because of its advantages.

ADVANTAGES

  • Breast milk easily digests
  • Protects against infection
  • It enhances bonding between mother and child
  • The process of breast feeding helps to delay fresh pregnancy in the mother
    • Prevents the monthly period which in turn, prevents anemia, since there would be
      no loss of blood
  • It reduces the risk of breast cancer and ovarian cyst
    • It cuts down on the cost of food in the family as artificial milk will not be included in
      the budget
    • It is environmental friendly as there would be no waste papers and cans to be
      discarded
  • It is hygienic and safe for consumption at all times
  • It reduces under 5 mortality

MOTHER'S PART

  • A lactating mother does not need to eat for two as most people say
  • She needs  to eat adequately and eat the right kinds of food_____________________

She needs to take in a lot of fluid

She is required to have enough rest

Those around should lend a helping hand and avoid stressing her or keeping her sad If all these measures are taken, there will be enough breast milk to satisfy the baby at all times, and the mother will remain healthy.

Topic:          MALARIA IN CHILDREN AND PREGNANT WOMEN

What is malaria: It is a parasitic infection that spreads from person to person through mosquito bite. The parasite lives in the blood cells. Incase of the female anopheles mosquito injects the parasite into the person, which it had sucked earlier from an infected person. The mosquito needs blood to develop the parasite so in the process of sucking blood and depositing the parasite malaria spread of malaria is worse where mosquito breeds e.g. stagnant water, dirty environment, bushes and trees with large leaves e.t.c

How does it present:    The  symptoms are not specific,  but a number

issues are involved e.g. fever, headache, generalized body pain, catarrh,

vomiting e.t.e.

In severe cases, convulsion yellowness of urines, yellowness of the eyes,

pale looks of the palms and eyes e.t.c

For   pregnant  women,   the   presentation   may  be   different   and   the

progression is very fast and could be very severe and fatal.

  • We can target mosquitoes and eliminate them from the
    system.
  • Malaria drugs could be bought off the shelf; most of them are less
    likely to harm the person.
  • For children, treatment should start at home and then present to
    the hospital

PREVENTION:    Malaria is 100% preventable.

Environmental management - clean drainage, dispose of empty can

Cover all stored buckets of water and tanks, so that mosquitoes

don't breed.

Use indoor residual spraying

Use insecticide treated nets - this is the best prevention method

COMPILATIONS  Compilations could be life threatening.

o         Severe Anemia requiring blood transfusion

o         Yellowness of the eyes that may look like hepatitis

o         Convulsion due to fever

o         Deficiency after recovering from convulsion

o         Blood sugar may be low

o         Shortage of blood due to depletion of red blood capsules

Drowning is death within 24 hrs of being submerged in water Near drowning is surviving after 24hrs of being submerged and rescued.

WHAT HAPPENS WHEN THE PERSON'S IS SUBMERGED.

  1. The person is unable to breathe
  2. The person may swallow a lot of water
    1. The person may expiate the water-ie breathing the water when
      gasping for air.
    2. The length of time the person stays in the water determines
      the extent of damage      in a near drown situation it could
      present with brain damage.

AND IN DROWNING; DEATH.

  1. Children should be well supervised
  2. Containers for water should be covered
  3. There should be a responsible adult to care for children
  4. Children at the swimming pool should be properly supervised.
    1. An adult who does not have the skills for swimming should
      not go to the deep area of the pool.
    2. One who has taken alcohol should not swim.

HOW TO RESUSCITATE A DROWNING PERSON

  1. Bring the person out of the water
  2. Shout for help
  3. Lie him/her face down
  4. Apply pressure to help the person vomit the water swallowed.

1.     Complete eradication of new infection

-      Zero fresh infection from sexual intercourse

-      Zero new infection from mother to child e.t.c

 

  1. Eradication of discrimination either by the infected individual or others
  2. Eradication of death arising from HIV infection.

Drugs are available and they are free. Treatment is given in three parts.

a.        Counseling

b.      Testing

c.      Treatment

HIV care can be obtained from UPTH, BMH and Health of the sick clinic at Nkpolu and all the general hospitals.

The effect of HIV on children is also being tackled. Children can now be born from by HIV infected mothers.

Technology has made it possible for HIV infected mothers to effectively breast feed for six months as baby friendly mothers and still breast feed for one year thereafter.

In a case where both spouses are positive what can be done with conception?

As long as the antiretroviral are being used, the CGPA count will increase and conception will take place without infection.

HIV is not a death warrant there are millions of people living with HIV and they are healthy as long as they take their medications.

 

Advice It is better to prevent than to treat   Always use insecticide treated mosquito nets
